The Results Nurse Navigator is responsible for identifying, investigating and communicating high risk test result findings to the appropriate medical and clinical members of the patients' care team and coordinating the patient care follow up throughout the continuum of care. He/she will apply nursing knowledge and skills and knowledge of the healthcare system to address and resolve gaps in communication of findings as related to ordered tests. The Results Nurse Navigator is a team member of the Care Coordination department within Population Health and also functions as a member of the patients' interdisciplinary care team. The Results Nurse Navigator reports to the Care Navigation Manager.
Experience working in a high reliability organization preferred
Prefer experience utilizing and documenting in EMR systems
Intermediate skills in using Word and Excel
Responsibilities:
*Utilize electronic tools and reporting to identify patients with high risk findings needing navigation
*Review EMRs for undocumented communication of high risk test result findings
*Ensure communication of high risk finding to the appropriate care team member
*Apply nursing knowledge, critical thinking and analytical skills within the scope of nursing practice
*Demonstrate a preoccupation with failure as it relates to a high reliability organization
*Coordinate appropriate patient referrals to services for continued care
*Assist patients with healthcare appointments
*Communicate effectively with members of the patient's interdisciplinary are team across the continuum of care, both internally and externally
*Serve as patient advocate to ensure a superior patient experience across the continuum
*Deliver care that is patient centric being sensitive to patient needs, cultures, health literacy and expectations
*Collaborate with the interdisciplinary team to evolve the role
