This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going tohttp://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. ["Major duties of the position include: Develops local policies and procedures in accordance to VHA and Medical Center policy, Federal, State and Local regulations, and Joint Commission requirements. Recruits Medical Foster Homes/Community Residential Care Centers and Medical Foster Home/CRC Caregivers, assesses the potential Caregivers capacity for care and oversees the administrative requirements involved in implementing Medical Foster Homes. Trains and orients new caregivers; including arranging semi-annual training sessions for caregivers. Facilitating the process of transitioning (not physically moving) Veterans into MFH's. Collects and maintains statistical and performance data, prepares reports and oversees performance improvement activities related to the Program. Provides Team Leadership for the Program, conducting regular oversight meetings to coordinate the provision of services in compliance with established policy and procedure. Provides Issue Briefs for reportable incidents to the VISN and CO and places Joint Patient Safety Reporting Coordinates inspections and serves as a member of the interdisciplinary inspection team, performing routine and special cause inspections of MFH/CRC; schedules visit, prepares deficiency letters, and collates plans of correction for team review. Recommends approval/disapproval of usage of these facilities based upon contractual requirements, patient care requirements, and Joint Commission standards. Investigates any report of harmful conditions on behalf of the patient and keeps management staff informed of findings. Assures that the MFH/CRC and its services interface with other VA and community programs to provide continuity of care to veterans. Coordinates activities with key inpatient and outpatient staff to ensure continuity of care to veterans. Evaluates the program and services and makes recommendations for new services based on outcome data and develops new initiatives and refines current program policies and procedures to meet the needs of Veterans enrolled in the program in an ever-changing environment. Coordinates educational and marketing services to Veterans, staff and the community regarding the MFH/CRC, Senior Companion, Community Adult Day and respite services. Represents the Program at appropriate service and facility meetings and establishes liaison relationships with community agencies pertinent to program operations. Screening/Assessment - Screens and assesses the Veteran and his/her caregivers' psychosocial needs and barriers in order to facilitate his or her access to appropriate VA and community resources. Through collaborative relationships with the Veteran, his or her family/caregiver and other VA programs and VA staff, the Medical Foster Home Coordinator coordinates the gathering of information vital to the process of setting appropriate care goals and time frames for achieving those goals. Assessment information may include, but is not limited to: Marital/family/social/care giver status, Medical status, Guardianship, POA, DPOA, other legal issues, Financial status, Use of entitlement programs, Strengths/Limitations Treatment Planning - From screening/assessment information, the Medical Foster Home Coordinator determines realistic goals with the Veteran and his/her caregiver, including such factors as a) the appropriate type of intervention and/or level of care, b) the setting conducive to providing such intervention or care, c) the need for intervention to allow for the least restrictive level of care, d) transportation options/barriers and e) the steps to obtaining resources (VA or community) that make the achievement of goals possible. Case Management- Provide case management services as needed to Veterans and their caregivers. As a lead coordinator for the CCICM program, the Medical Foster Home Coordinator may be assigned as a lead coordinator for Veterans outside of the Medical Foster Home program. Referral to service providers - To facilitate the continuity of a Veteran's care to maintain optimum functioning and quality of life, the Medical Foster Home Coordinator will arrange referrals to needed VA and/or community resources (e.g., home health care, services available through area agencies on aging, community support groups, hospice care). Crisis Intervention - Assess and intervene in any medical or psychiatric crises presented by the Veteran and participate in developing crisis management and safety plans to minimize the event of future crises.
