The Social Worker Program Coordinator is administratively responsible for the clinical programming and daily operations of the Program of Comprehensive Assistance for Family Caregivers and may serve as back-up to the facility CSP Program Manager. The Coordinator will function in support of caregivers, Veterans and family members throughout the Medical Center and in the community and will support the development and implementation of Caregiver Support Program services. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website (www.cswe.org) to verify if that social work degree meets the accreditation standards for a master of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j. Preferred Experience: - Certified Lean Six Sigma and demonstrates an ability to conduct performance improvement efforts based on systems engineering principles. - Experience working in a community setting conducting screenings, assessments and demonstrating high level of independent problem solving skills and crisis intervention techniques. - Strong outreach and public speaking skills. Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates Social Worker (Program Coordinator), GS-12 Experience, Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition, the candidate must demonstrate the professional KSAs in subparagraph (b) below. 1. Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. 2. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. 3. Ability to supervise multidisciplinary staff assigned to the program. 4. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). 5. Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher- level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. Walking long distance throughout the medical center, on units and between divisions is required. Conducting patient encounters in a community setting or in the patient's home environment may be required. Position often places the incumbent in a position of stress. Due to the nature of the work, the incumbent may come in contact with Veteran patients who are confused, intoxicated or have a contagious condition. Ability to drive a car and have a valid state license is required. Exposure to dangerous situations in the community from traffic accidents to crime is also possible. ["The incumbent serves as a Social Worker Program Coordinator, locally titled PCAFC Program Coordinator for Augusta, Georgia, which provides a state-of-the-art continuum of services, education and resources to caregivers of Veterans with complex needs. The incumbent oversees service delivery and coordination of care activities for family caregivers who provide personal care services to eligible Veterans regardless of period of service. Major duties include, but are not limited to the following: The Coordinator develops and implements the PCAFC specific to his/her area of assignment and insures compliance and collaboration with local facility and the VA Central Office Caregiver Support Program initiatives in concert with the facility CSP Program Manager. Assignment will include cross coverage within the PCAFC and/or PGCSS teams. Travel maybe required to approximately 100 miles of the local VAMC The Coordinator provides the full range of supervisory duties which would include responsibility for assignment of work to be performed; performance evaluations; selection of staff, training of staff, and recommendation of awards, advancements, and when appropriate, disciplinary action. Tasks include supervising CSP PCAFC staff, tracking consults and workload, assisting, covering assignment areas of PCAFC staff, conducting frequent needs assessments to implement education, wellness, and support groups. In addition, the Coordinator must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations. The Coordinator administers information and analytical systems to evaluate and enhance the quality of service provided to caregivers, families and Veterans. Develops and implements information systems to track service activities including visits, referrals, caregiver demographics, equipment utilization and caregiver and Veteran satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. The Coordinator provides ongoing consultation and support to colleagues regarding caregiver support. Develops and facilitates caregiver support groups (face-to-face or VA video Connect (VVC) and educational programs. The Coordinator coordinates with other disciplines within the medical center to maintain the Veteran Assessment Clinic, for the purpose of providing a comprehensive assessment for the veterans and caregivers in the PCAFC and seeking assistance from the program. The Coordinator serves as a liaison between the VA and community agencies. Participates in caregiver support activities in the local community and state to promote and recognize the role of the caregiver. Participates on state and local caregiver support committees. Promotes the VA Caregiver Support Program in coordination with the local VA Public Affairs Officer through interviews with local print, voice and television media. The Coordinator, in collaboration with the with other clinical staff and program managers, will establish a caregiver support protocol (resources, education and training) sensitive to the issues of the caregivers who are providing care to our youngest returning Veterans. The Coordinator will develop a data monitoring system to measure the efficacy of the Program of Comprehensive Assistance for Family Caregivers Program. The Coordinator collects, analyzes and reports program data to local management, VISN Caregiver Support Coordinator Lead and the National Caregiver Support Program, Care Management and Social Work Service, Office of Patient Care Services. Evaluates program progress and outcomes and utilizes data to continuously modify and improve the program and processes. The Coordinator performs a wide range of services and provides technical assistance to state, local, and private agencies, and universities. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information. In addition, the Coordinator uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements.
