The VA Ann Arbor Healthcare System is seeking a Registered Nurse (Risk Manager/Patient Safety Specialist) to be a member of the Patient Safety Team. The Registered Nurse, Risk Manager/Patient Safety Specialist, focuses on Risk Management, Patient Safety and High Reliable Organizations (HRO) processes. The Risk Manager/Patient Safety Specialist functions in collaboration with all clinical programs to ensure the delivery of safer care and works with the Patient Safety Manager to assure continuous survey readiness, compliance with the national office program requirements and compliance with National Patient Safety Goals. The incumbent communicates with appropriate staff regarding identification of risk and safety issues. The incumbent completes daily reviews of electronic incident reports and/or occurrence screens. The Risk Manager/Patient Safety Specialist is responsible for analyzing data from monitoring activities and presenting it to the appropriate audience in useable information formats. The Risk Manager/Patient Safety Specialist manages databases (Peer Review, Torts, Administrative Board of Investigations, Fact Finding, electronic incident reporting system , SPOT, PSAT, Alerts and Recalls, etc.) including entering data and pulling reports to assist the facility in decreasing adverse events. The incumbent participates in activities related to patient safety such as participation/lead/advise on Root Cause Analysis (RCA) teams, completion of Healthcare Failure Mode Effects Analysis (HFMEA), tracking of alerts and recalls, action plans related to either potential or adverse events, implementation of CERNER. The incumbent tracks action plans and assures sustained change. The incumbent utilizes constructive problem solving and management of conflict related to patient safety within the facility.
