The Clinical Pharmacy Technician will routinely be assigned specific clinical work to include laboratory value monitoring, medical chart documentation, drug utilization evaluation monitoring, pharmacoeconomic reviews, medication utilization monitoring, telephone logistical support, supplemental patient education communication, and assisting with medication ordering. The coordination and provision of clinical pharmacy services to the patients receiving anticoagulation therapy through the Centralized Anticoagulation Service Hub. Major Duties: Conduct patient interviews, including medication history, anticoagulation history, and effectiveness of anticoagulation, need for medication refills and investigating causes of therapeutic irregularities. Communicate changes in anticoagulant medication and/or dose with patient if applicable. Accurately documents all communication in the patient's electronic record. This includes reporting of adverse drug reactions and incident reporting. Participates (in conjunction with clinical pharmacists) in medication utilization evaluations, quality improvement projects, and quality reporting as assigned through Hub Clinical Pharmacy Program Manager. Attends and actively participates in all clinical pharmacy activities as directed by Hub Clinical Pharmacy Program Manager, i.e., staff meetings, staff development. Triage phone calls and voicemail messages from internal and external customers. Assist with administrative tasks related to Professional Practice Evaluations. Manages technical assignments of anticoagulation clinic in order to assure continuous and consistent coverage of the clinic. Monitors patient scheduling to assure appropriate patient follow-up and time to next clinic availability. Provides training and resources for clinical staff who are new to Hub group, as applicable.
